Affinity Care Hospice offers patients and their families compassionate, professional palliative and hospice care wherever they call home, We provide the highest quality of services, clinical excellence and the most innovative care possible to help patients maintain their quality of life, minimize crises, and ensure consistent, compassionate support.

Our approach centers on service intensity and continuity of care—medically, emotionally and spiritually. This includes a dedicated team of nurses, CNAs, social workers, chaplains, certified music therapists, supportive doulas and volunteers. Our unique model of care allows for a seamless transition for patients as their needs shift and change. We understand that this time can be overwhelming for both patients and their loved ones. Our goal is to provide comfort, clarity, and peace—so families can spend meaningful time together, supported by a team they can trust.
